Conference Sneak Peek: Electronic Health Records
One of the biggest challenges for our industry is the adoption of electronic health records. The Federal government and a host of policy groups and health information organizations are working to make widespread, interoperable EHRs a reality. Come to the Directions conference and you'll hear Doc DeVore present a State of the Industry session on this topic. Mr. DeVore is Director of Research and Development for MDI Technologies, and serves as Chairman of the IT Committee, NASL (National Association for the Support of Long Term Care) and is a member of the NASL Executive Board, NASL Standards Workgroup, HL7 Long Term Care EHR Profile Workgroup and the NCPDP (Nat'l Council for Prescription Drug Programs) Workgroup 14 (EHR/HL7 TG).

McKesson Users: NPI in Electronic Claims
We recently became aware of a situation that may affect you when filing electronic claims through the McKesson clearinghouse. If your Intermediary has notified you that you may submit your claims using only your NPI (i.e. your legacy provider number is no longer necessary on claims), then your NPI must appear in 2 locations in your transmission file to McKesson. We have made the necessary programming adjustments to accommodate this requirement and the correction is available on our secure site at https://support.monetteinc.com/version15. This change has no affect on you if your Intermediary is still processing claims in a 'dual-mode' - accepting both your NPI and legacy provider numbers. This change also has no effect on users filing claims electronically using any other method.

Proper Use of NPI in ULTRACare
We have heard that some users have mistakenly thought it necessary to enter their facility's NPI in the field labeled NPI on their Insurance Code setup screen. This field is for future use when the Insurance Intermediary provides their NPI to identify them. Until that time, it is not necessary to change the contents of this field in your setup.

UB-04 REMINDERS
To print a pre-printed color UB-04 form using ULTRACare and a laser printer, you must set the printer device code (in Admissions System Codes) for program ID "PTUB92" to QPQ:53. This tells the printer to ignore software print margins, but most laser printers are limited to printing 80 characters across the page.

The National Uniform Billing Committee (NUBC) reports "some laser printers are having difficulty meeting the print specification of the UB-04 form. The UB-04 form and the UB-92 contain identical margin specifications. Both forms are 82 characters across. To accommodate the 80 character limitation of some laser printers, many users of the UB-92 form developed workarounds that basically "cheated" on the printing layout. This was commonly accomplished by starting in the second position and ending in the 80th position, basically ignoring the first column on the left and the last column on the right. The UB-92 had no critical data elements in these fields."

We have adjusted the printing of the UB-04 form on a regular laser printer to the best of our ability to get the information to appear inside the boxes. Due to the layout of the new form, there are issues related to the margins:

  • A laser printer has a 4mm margin on the left and right, so the contents of certain boxes will print outside the box.
  • On the right, the following will truncate the last digit: box 4 - type of bill, box 41 - value amounts, and 76 & 78 - ID number for qualifier.

The NUBC further recommends that "in order to meet the UB-04 print specifications, users should utilize laser printers that have "edge-to-edge" print capability (4 mm margins on the left and right) or wide carriage impact printers (dot-matrix or line printers)." Reportedly, the HP9000 series printers have the ability to Turn On Edge-to-Edge printing (1mm vs. 4mm) margins. This ability comes at a cost of roughly $3,000.

KX Modifier Not a Free Pass to Exceed Therapy Cap
CMS cautions providers about over utilizing the KX modifier for therapy services that exceed the Medicare B therapy cap. The KX modifier should only be used for therapy services beyond the cap that are medically necessary. If therapy services are provided and found NOT to be medically necessary, benefits will be denied and the beneficiary would be held liable for the charges.
